ACO Clinical Quality Consultant

Position Summary:

The ACO Clinical Quality Consultant is responsible for implementing and coordinating OneCare’s quality improvement program in collaboration with the Sr. Clinical and Quality Specialist. The incumbent will use data driven methodology to work with clinical leadership to set priorities for improvements aligned to ongoing strategic initiatives. The incumbent will provide leadership to the clinical unit team to articulate the vision and values of the organization. The incumbent will provide clinical leadership and support to the clinical unit team and providers within their assigned health service areas in order to support the OneCare ACO’s integrated health care delivery model that is centered on the patient and family. The incumbent will participate in the Quality Improvement Committee, sharing best practices and tools and materials and capacity-building peer exchanges.  She/he will also work within regions of the network to identify regional shared goals and work within them to provide expertise on quality improvement strategies. The incumbent will work with the clinical unit and within his/her assigned health service area(s) as part of an Accountable Care Team in order to assist with the creation of a high value health care delivery system that meets the Triple Aim of optimizing the health, total cost of care, and patient experience of the population. The incumbent will be accountable for focusing on five (5) improvement categories, measuring and monitoring the success in each: 1) enhancing clinical and quality outcomes, 2) improving beneficiary engagement, 3) utilizing clinical practice technology provided by the ACO, 4) improving patient experience/satisfaction, and 5) controlling costs through improved health care delivery strategies. The incumbent is a key contributor to efforts aimed at meeting OneCare ACO’s clinical priorities and goals and the strategic priorities of the local health service area(s) they are assigned. Knowledge of Vermont health systems, public health programs and organizations is critical. The ability to establish and maintain effective and productive working relationships with public and private organizations, practices, partnerships, and communities is vital.

Education:

Registered Nurse licensed in the state of Vermont Required. Bachelor's degree in nursing or health care administration or related strongly preferred. Other licensed health care professionals may be considered, i.e. MSW, LICSW, LMHC. Certification in Health Care Quality (CPHQ) preferred.

Experience:

Three (3) to five (5) years of varied clinical experience and an additional three to five years of experience in an accountable or managed care, medical home, health plan or quality improvement setting. Direct experience using and reporting from clinical care coordination software. Three (3) to five (5) years in a project management or similar setting highly desirable.
